this information the consultants come up with will still be rife with potential issues, but at least its something. if they can say 'the magic number of residents per year is in this ballpark' that is something. ASTRO can afford it.

I can’t imagine what a “consultant”/ someone who knows 0 abt radiation could bring to the table, other than reaffirming the obvious.

Consultants are a dime a dozen. They may all say the same things but some may make the situation less dire than others. Astro will pick the one that minimizes the issue or shifts the focus to something else.

Hospital CEO knows this well. They pay consultants to review physician contracts and give them an expert opinion whether physician salary is legally "safe". No way the CEO could possibly wink-wink-nudge-nudge the consultant as to what they want that "safe" number to be.

Want to give somebody a bad deal but don't want the blame for it? Hire a consultant. Hey, my hands are tied. We'd sure like to keep you at your current pay, but we just got a report that says it's too risky, so unfortunately we have to cut your pay 20%. Consultant says so.

This is precisely what happens. They assign the blame and they have a “objective” party to back it up. Hospital wants to get rid of a group? Send in the consultants. Wanna cut some money losing department…hire the consultant. That’s not to say they cannot be oused to our advantage…it’s just that we suck at doing anything that’s in our best interest.

i mean sure I get this criticism.

but ultimately..... like what do people expect to happen? that programs just decide to close up shop?

this is the first legitimate step to any sort of action.

the perfect (which can't even be defined) can't be the enemy of the good.

the major criticism is that it took so long, but I am glad that they are taking this seriously.
To prevent further job market problems we should match 120/year. To improve the job market we should match less than 120/year. The less we match under that number the faster things will improve.

Regardless of what ASTRO ultimately "finds", it is going to **** a bunch of people off. The probably assume that having an "outside consultant" do the research will afford a protective blanket of "impartiality".

Little more proactive…say 75/year to make up for the years of expansion.

Maybe that’s good for society because then academia can warp speed FLASH therapy and finally drive the silver steak into the heart of all RO like they’ve always wanted to.

I’ll go extreme and say we need to subtract jobs from practicing physicians. I’ll likely be sacraficing my own livelihood but I’m down for the cause… viva La revolucion!
 

On the job ad it says "*If you do not see the pay range for this position listed, you are on a third-party job board. All our pay ranges are posted on our careers site www.centura.org/careers. Please visit our careers’ page and apply today!"

So, when you go to Centura's website, you find the ad. Ok, fine.


Except you get this:

Minimum Pay USD $17.00/Hr. Hiring Maximum Pay USD $12.15/Hr.

A nonsensical pay range. Even if it made sense, obviously fake numbers for a rad onc and laughing in the face of the law.
But wait, there's more. If you actually apply for this job, the application forces you to enter your "expected pay" as a number. There is no option to enter a character other than a number to put a range, or type something like negotiable or depends on volume or something like that. The only way to submit the application is type a number like 500000 or something like that. So they are advertising a job in a state that has a law that requires employers to disclose pay ranges and they have weaseled their way around it and forced applicants to throw the first number out (which any used car dealer will tell you you NEVER do). LOL. I guarantee you some admin is rank sorting applications by that column.

Ya it is posted every so often, maybe 2 or 3 times in past 5 years? Have never seen it advertise the compensation or technical buy in before.

It is. It was offered back in the fall. I am not sure what they are looking for, but they seem to be taking their time. I applied and actually got a very kind response that basically said they inundated with applicants. In other words, I suspect your C.V. needs to be littered with rockstar name recognition. I am surprised by the compensation. I had heard it was significantly lower than that somewhere else.

This is one of the biggest lies I was ever told. When I started my first job out of residency at a hospital, a disgruntled med onc colleague explained to me how we were in fact private practice doctors and needed to act as such. Unfortunately his fantasy led to numerous unrealistic expectations on my part and lessons learned the hard way about what it means to work for a hospital.

There has been recent turnover however, would check private forum for more info, always good to talk to the recent docs that have left a practice

Ya it’s kinda hard to tell with the different variations. Very few strictly private practice jobs where the clinic/machines are owned outright by a rad onc group. Then you have the small to very large medical groups that have rad onc, which is technically private practice. And then there are the ultra large groups like the ones that staff Kaiser that are technically also private practice. A lot of the time the ads aren’t really specific in terms if you will be working for a group or a hospital/health system. And, yes, Portsmouth OH has excellent meth.

Correct, @fiji128 is missing that group, major one is McKesson/US oncology which also bought former vantage practices in 2015, essentially leading to a hybrid where the docs are PP but essentially JV or have McKesson/US oncology manage the technical side of the practices where they also own a major stake. There's also the 21C/Genesis care model, i get a sense that's also closer to private than "employed"
 
Genesis is employed. Don’t get the say in day-to day operations.
However , their compensation structures does include some “shares”

Will never fail to bemuse me that almost all rad oncs have gotten to a point where they ostensibly say "You know what, rather than me having any access to the technical profit that I generate, I would rather a concentrated coterie of non-physicians reap the financial rewards of my work, sweat, and liability that I take on every day."

Truthfully if I was fresh out of residency I’d consider it. Non Phoenix Arizona is ridiculously good for out doors type of stuff but you have to be into that. If you need to be around hipsters and what not, ya, this job is not for you.

I had a phone call with them a while ago. My impression was it was yet another rural place trying to pull some shenanigans with unrealistic expectations for recruiting. There was a palpable negative tone from the clinic manager that threw up some red flags to me about how I might be treated there. It's the same old story. They want somebody who will come there, work 5 days a week, never leave their very isolated community, and be employed for MGMA median ($550k, which is what they are offering). If they get a whiff you want to work 3-4 days and go back to Scottsdale on the weekends, I doubt they are going to bring you out. I was not invited out, and I'm not sure would have gone if I were.

Do not believe the "potential" and "bonus" quoted by these kinds of places. They have so many ways of keeping you at your base.

I would say, at a minimum:

Independent billing or $800k base with 5 year guarantee, 4 days on site with a 5th day remote coverage, 40 days PTO, no payback clause for leaving.

Otherwise jobs like this are a non-starter.

There was a palpable negative tone from the clinic manager that threw up some red flags to me about how I might be treated there. It's the same old story. They want somebody who will come there, work 5 days a week, never leave their very isolated community, and be employed for MGMA median ($550k, which is what they are offering). If they get a whiff you want to work 3-4 days and go back to Scottsdale on the weekends, I doubt they are going to bring you out. I was not invited out, and I'm not sure would have gone if I were.

Do not believe the "potential" and "bonus" quoted by these kinds of places. They have so many ways of keeping you at your base.

I would say, at a minimum:

Independent billing or $800k base with 5 year guarantee, 4 days on site with a 5th day remote coverage, 40 days PTO, no payback clause for leaving.

Otherwise jobs like this are a non-starter.

All that is reasonable, but you can’t base someone at $800k, it’s far above FMV. You can structure it to make it work, but if you’ve talked to them, then I guess that’s what it was. I was told that one could reasonable get a package close to a mil.

For 2M? Wtf is so special about Show Low. Is it the next Asheville or something
At over 6000 ft elevation, has a Mediterranean climate. If builders are putting in spec houses at 2 mil plus, no matter how it looks, reasonable amount of money floating around the area.
