A 77 year old woman with a 7 year history of hypertension comes for a follow up exam. She has had intermittent atrial fibrillation despite cardioversion 2 months ago. She has no history of cerebral infaction or TIA. Current meds include hydrochlorothiazide and enalapril. Her temp is 98.6 (37C), pulse is 104/min and irregularly irregular, resp are 14/min and BP is 170/90. Exam shows no other abnormalities. Which of the following is the most appropriate pharmacotherapy to prevent cerebral infact in this patient?

A) Aspirin
B) Clopidogrel
C) Dipyridamole
D) Warfarin
E) No pharmacotherapy needed

Also, does anyone have the answer key and explanations to the NBME Medicine assessment forms ?
