Like they are a community program where most of the residents do not do fellowship but also are almost entirely USMDs. My understanding is that they are highly regarded in primary care and also psych but how difficult would it be to specialize from residency there? How is the reputation?

You've asked the same/similar question on several threads. I'll give you the same answer.

The best predictor of whether you'll be able to get a fellowship in a field you're interested in is prior graduates of that program doing the same. A program where graduates do not go into fellowships is unlikely to have the support mechanisms to help you achieve that goal. After a track record of success, the next most important item is a fellowship in the field of interest.

You seem overly focused on reputation, which is nebulous and unmeasurable. CHA seems like a decent program, you'd probably get good training there.

Unless you want a fellowship in Nephrology. They can't give those away, and spots are filled by people who haven't completed an IM residency yet. That, you can get from any program, at all. Endo and ID are also less competitive, but fill rates for both have improved recently. All of the rest of the fields are unlikely to be successful.
