General Should I disclose that I suffer from anxiety in health professional school enrollment forms?

Hello everyone,
I’m an incoming M1. One of the health forms that I need to complete has questions about whether you’ve had so and so symptoms (vision problem, heart problems etc). They ask in one section whether you suffer from anxiety and panic attacks. I do, but I am wary of disclosing it on the forms. Thoughts?

If you feel that you will need accommodations for anxiety, it will be good to do so.

I don’t feel that I would need accommodations, my worry was it being in my record and thus seen by the dean and thus residency programs, and thus affecting match. So this is NOT a concern?

This is NOT a concern. Read up on HiPAA.
