I have finished my first year at an osteopathic medical school, now I am faced with a dilemma. I have to choose between separating my family and stopping medical school to live overseas for 2-4+ years.

I don't know what to do!

What happens if I withdraw and try to re-apply in a few years?

FYI, when I started attending school, we expected to be stable in the area while I attended school, and we just found out that that will not be the case.

Any advice would be greatly appreciated.

This is out of my area of expertise, but I'd suggest talking to your school. Many schools will allow you a leave of absence / deferrment. I doubt they would do so for 4 years, but maybe 2. Or, they may be willing to "promise" you a spot when you return, although they might require that you start over with the first year.

Your best shot will be with the school you are currently in. The better your current perofrmance is, the better your chances.
