Pain Management Fellowship Program at UM Pain Center
The Pain Management Fellowship (PMF) is an advanced clinical training program in pain medicine, within the Department of Anesthesiology. A full-time board-certified anesthesiologist, certified in pain management serves as Medical Director. Dr. Fred Furgang, is dedicated to the PMF and provides supervision of clinical services and resident training. The Medical Director is assisted by fellowship trained faculty. The PMF comprises three fellowship positions, each comprising a 4-month rotation at: Jackson Memorial Hospital Pain Management Center, the U/M Sylvester Cancer Hospital Pain Center and the Veterans Administration Hospital.

Jackson Memorial Hospital

Acute Pain Control Service
The management of acute pain includes postoperative pain management at JMH and the Ryder Trauma Center, and responding to consultation from other medical services for the control of acute pain for both outpatients and inpatients. Under the supervision of Dr. Furgang and other pain faculty, the fellow leads a team of residents rotating on the Pain Management Service. The fellow is responsible for organizing daily rounds that includes the evaluation and treatment of in-house patients on the acute pain service. Techniques include PCA, single-shot epidural and intra-thecal opioids, continuous epidural infusion of local anesthetic/opioid combinations and PCEA, continuous interpleural analgesia, and peripheral nerve blocks. The team is responsible for determining the efficacy of the therapy administered and making appropriate changes. The fellow is responsible for ongoing monitoring of the effectiveness of various modalities and identification of any administrative or clinical problems that arise.

Chronic and Cancer Pain
The Pain Management Center at JMH is a high volume chronic pain clinic which stresses the medical and interventional management of chronic and cancer pain. Jackson Memorial Hospital provides a full spectrum of physician and ancillary consultative services, which are frequently employed on a referral basis by the pain management trainees. The fellow has the opportunity to participate in the scheduling of patients' initial consultation, diagnosis and treatment plan development. Patients suffering from a large variety of pain problems are seen daily in the PMC. Problem cases are discussed in small group format. Follow up is frequently discussed with attending to broaden input.

The Miami Veterans Administration Medical Center

Dr. Vivek Mehta, a board-certified anesthesiologist also certified in pain management, has established a full-spectrum pain center at the VAMC, and frequently tutors the fellows in the use of multiple techniques. In addition, the VA patient represents a unique population which greatly contributes to the fellowship experience. Open communication with the VAMC clinic permits the fellow to participate in interesting cases and procedures.

University of Miami Comprehensive Cancer Center Pain Clinic

The Sylvester Center is a nationally recognized center for the treatment of cancer. As a result, the Pain Center there has an extraordinary selection of patients with cancer pain. Under the direct supervision of Dr. Dennis Patin, a board-certified anesthesiologist also certified in pain management, the fellow receives extensive experience in the management of patients with cancer pain, although other patients with non-cancer chronic pain are referred to the center. The full range of diagnostic and therapeutic options is available to the fellow in training at SCCC.
