I was wondering how significant are extracurricular activities (clubs and leadership positions) in an oto application. I know that they are important for election to AOA, but beyond that how important are they?

I doubt they're even that important for AOA. I think the clubs are just meant as a diversion from your studies so you have something else (productive and academically oriented) to do.

Extracurrics matter very little in getting into residency. Beyonds grades and USMLE, focus on research.
