Doubt on encapsulation of benign tumor?

Its is said that benign tumours are well-circumsribed and encapsulated? What does encapsulated means? Does it mean the benign mass is surrounded by a capsule? If yes then how the capsule is formed and why malignant tumor lacks it? Or how it (the capsule) is lost when a benign mass turns malignant?
